Requirements for Positions
Clinical positions have certain requirements due to governmental regulations and/or Life Choice policy. These requirements are intended to provide the highest level of protection for our patients and their loved ones. Therefore, the following items (consents for some items are required in the application process) are required for any applicant in a clinical role:
A reliable automobile –You must have use of this vehicle daily as you will travel among facilities or homes in your area.
A valid current state driver’s license.
Valid car insurance – There must not be any interruptions in your coverage once you are hired.
Valid clinical license or certification – All professional licenses/certifications are verified through a primary source.
Criminal Background Clearance – May be state and/or federal in nature. You will present your authorization for this clearance to Human Resources as part of the application process.
Motor Vehicle Clearance – We verify that all applicants have a valid driver’s license and an acceptable driving record.
Educational Background Clearance – Life Choice Hospice verifies educational credentials for selected positions.
Three professional references – We prefer former supervisors
Two forms of identification (detailed below) that will be validated to ensure you are legally allowed to work in the United States.
Clearance from the Office of Inspector General’s non-exclusion list.
Physical requirements (detailed below).
Drug testing – Life Choice Hospice may test for the presence of drugs prior to your start date and at any point randomly thereafter.
Validation that you are not currently bound to any employment contract prior to accepting an offer from Life Choice Hospice. You will sign a statement we’ve prepared regarding this issue.
Once You Have Accepted a Position - What to Bring to Your Meeting with Human Resources Prior to Your Start Date
In the event that you are extended offers of employment, please be advised that there are the following items must be satisfied prior to starting employment: An appointment will with a Human Resources representative is required. At this appointment please be prepared to bring original copies (photocopies cannot be accepted) of the following items:
A passport or a state driver’s license and either your social security card or original copy of your birth certificate. Other forms of ID may be presented after speaking with Human Resources
State Drivers license
A valid car insurance card – this must be presented prior to hire and then with every renewal thereafter.
Proof of Residency in your state for at least two years – an old document showing your address and the date will suffice
An original hard copy of the display portion (the large copy – not the wallet portion) of your professional license or your certification must be presented to Human Resources. Human Resources will verify your credentials to ensure that they are in good standing.
Physical Requirements
A physical exam within the last year. If you haven’t had a physical this recently, it is advisable that you schedule a physical immediately with our Occupational Health panel physician group. Please call Human Resources at 800-557-7570 to schedule this physical at an occupational health provider in your area. This document must be received prior to your start date.
Tuberculosis testing: Life Choice Hospice require a two step process or a Chest X-ray within the last year. If you have had TB testing this year, please provide a copy. That PPD can count as one of the two required tests, but you will still need another prior to your start date. You can come to the office to receive the injection during normal business hours. A Life Choice nurse can read the results two to three days after the injection. You must have this completed prior to the start date.
Rubeolla and Rubella – Depending upon your state of employment, certain states require that individuals working in a clinical capacity have titers that prevent the transmission of these diseases.
