Aging is a well orchestrated symphony as if compared to a rose.
The rose in its budding splendor gathers beams of light and energy to proliferate.
Its roots stretch and sprout while its foliage unfurls rejoicing to celebrate its beauty, the bud.
The bud, the soft, petal pink perfume brings pleasure to those in its realm.
The petals are so delicate we nurture, will endure its reign of splendor.
For after the rose has reached its life’s purpose, its petals slowly dry, wither and exfoliate,
casting itself to the ground from which it sprouted.
Yet in all its stages of glory, the caretaker (Our Heavenly Maker) tended the rose to reach its grandeur
and it has served well reaching fulfillment of life.
Each rose has budded, blossomed and yielded to create our existence today.

Our caring team visits patients as much or as little
as needed. We are always available to provide support
and instruct caregivers 24 hours a day, seven days a
week in any setting:
Reimbursement
Hospice services – including medications, medical supplies and medical equipment relating to the illness and included in the Hospice Plan of Care – are covered by Medicare and Medicaid.
Physicians
Life Choice Hospice is an enhancement to the care and
services you offer your patients and their families.

Support Services
Life Choice Hospice provides bereavement support to families through letters, cards and telephone conferences. This support continues for 13 months after the passing of a loved one. An evaluation is provided at the beginning and end of the process and you may discontinue support at any time. We also offer referral services for those
who may need more support.

Celebration of Life
Life Choice never stops caring about those who have
passed and their families. We celebrate the life of
every patient and family member with an annual "Celebration
of Life" memorial service to remember and honor
those who have passed during the previous year.
The "Celebration of Life" memorial service
includes music, an inspirational message and a special
candle lighting ceremony in memory of each loved one
lost. Friends and family receive special invitations
and a reception follows the service to continue the
"Celebration of Life."
