All Life Choice Hospice Locations

Certified Nursing Assistants (CNAs) are needed for both PA and NJ service areas.  CNAs at Life Choice rotate every other weekend and care for patients in the nursing home, assisted living or home environment.  Hours of work are all day shift hours.  Must be a CNA certified in your state of residence with reliable transportation.  Must be able to pass organizational clearances.

As a hospice Admissions RN you will admit patients onto our service and develop their initial plan of care.  You will also cover referrals and perform day-after-admission visits.  This will enable you to interact with patients, families and facility staff members in the process of educating regarding the hospice benefit and incorporating the person within the patient into the care plan.  Other elements of the position include a three (3) twelve (12) hour day work week working 10A to 10P.

Job Duties:

As an On-call Hospice RN, you will work 5 PM - 11 PM on both Thursday and Friday and 7 AM to 7 PM each Saturday and Sunday.  You will function as an integral part of our care team and this role will serve to strengthen our on-call team ensuring a high level of responsiveness and quality clinical care.  This position is designed to improve our responsiveness to clinical situations, reduce the territory for our present on call staff and increase our staffing during peak admission hours.  You will be responsible for responding to clinical situations representing the organization after normal business hours including acute patient issues, admissions, referrals and making day after visits for newly admitted patients. With our integrated electronic patient record, our on call RNs have access to the entire patient record at any moment, including the complete medication profile.  At Life Choice Hospice we make a strong effort to allow you to be the nurse you envisioned when you entered the profession by empowering you to perform excellence in care. Requirements:

Intake Manager (LPN)

Job Duties:

As our Intake Manager, you will coordinate the referral process ensuring the foundation is laid for a smooth intake of a new patient.  This position is vital to providing the excellence in customer service and responsiveness that Life Choice embodies.  In this role, you will receive and track referrals and maintain communication among the respective parties involved in the admission process.  You will schedule nursing for initial evaluations when appropriate and coordinate with our interdisciplinary team (Social Work, Pastoral Care, Volunteers, CNA’s, DME, and Billing) to initiate service.   At times, you will also interact with patients, families and facility staff members while educating them regarding the hospice benefit.  We have a first rate on-boarding process for new employees and invest in staff education for all our employees. 

Requirements:
The ideal candidate will have 1 to 2 years of hospice, LTC, or Acute Care experience with the focus of the position on admissions. He/she must be technologically savvy, possess a devotion to patient centered care, and exude a warm engaging interpersonal style. Valid professional LPN licensure in the state of Pennsylvania is also required.

 


Bereavement Coordinator

Job Duties:

The Bereavement Coordinator will provide support and counseling to the Patient’s family, facility personnel, fellow LCH employees and community members. You will serve as resource to facilities, families and our own professionals regarding grief and loss issues and provide education to the public in areas of grief and bereavement. The role involves assessing the bereavement needs of the Hospice family utilizing information obtained from the interdisciplinary group, and sometimes through direct contact with the family prior to the patient's death, then developing and implementing a bereavement plan of care for each Hospice family whose loved one died.   You will Coordinate Mailings with our Volunteers, assist with monthly BV audits and help prepare for the Annual Memorial Service. This individual will also have the opportunity to assist with pre-bereavement calls and family/community/facility support as needed.

 The position will also provide administrative support to the Pastoral Care Team in the form of tracking deaths, community support services, patient calls and letters sent.  You will maintain accurate and timely documentation of Plan of Care Letters and Quarterly mailings in our electronic information system.  The role requires you prepare for team meetings, such as printing weekly Post Death Assessments and creating task schedule in our contact management system.

Requirements:

The ideal candidate will have 1 to 2 years of counseling experience and be willing to travel occasionally in their local area. Additionally, the ideal candidate will possess exceptional counseling skills, a devotion to patient centered care, and a warm engaging interpersonal style.  Additionally:

  • MDiv or Masters in Theology  
  • A reliable automobile (mileage is reimbursed)

Job Duties:

As our Patient Data Coordinator, you will be responsible for ensuring our medical records are maintained in an orderly fashion and perform day-to-day maintenance so that individual records can be readily retrieved.  In this role, you will receive and track requests for Durable Medical Equipment (DME) and maintain communication among the respective parties involved.  You will work closely with our existing vendors and develop relationships with new vendors.  You will also be responsible for coordinating with our Billing department to ensure terms of the contract are fulfilled and that the product is returned upon completion of use.  You will be a key team member in coordinating activity and collaborating with the clinical team to meet patient needs.

This is Part Time position three days a week, eight hours per day.  We offer some flexibility in terms of which days a week you choose to work.

Hospice Social Worker

Job Duties:

The Hospice Social Worker plays a key role in assisting the patient and family during their journey at the end of life. The hospice Social Worker visits our patients, primarily living in the nursing home environment, and collaborates with the patient and family to promote the psychosocial well being and address coping issues that may arise. The hospice Social Worker also assists in coordination issues regarding financial assistance and funeral home procurement. To achieve these goals the hospice Social Worker is intimately involved in a collaborative relationship with the patient, family and the entire interdisciplinary hospice team.

This role allows you to utilize your clinical skills while you develop meaningful relationships with your clients, their families, and with other members of our extraordinary team.   You will be empowered to perform excellence in care through realistic ratios enabling you to spend quality time with patients.
This position will serve our Delaware County patients out of our PA office.

Requirements:

The ideal candidate will have 1 to 2 years of hospice experience and be willing to travel in their local area.  Additionally, the ideal candidate will possess exceptional clinical skills, a devotion to patient centered care, and a warm engaging interpersonal style.  Additionally:

  • A Masters is Social Work is highly preferred, particularly at a school accredited by the Council on Social Work Education

As a Hospice Pastoral Care Counselor, you will follow your patients over the course of their end of life journey supporting the spiritual needs of hospice patients and their families.  The Chaplain travels throughout their territory to meet with patients in skilled nursing facilities, assisted living facilities or private homes and renders spiritual support. You will perform spiritual assessments and also work with the other disciplines of our team to provide excellence in care from a multi-disciplinary approach.

In this role you will utilize your counseling skills while you develop meaningful relationships with your clients, their families, and with other members of our extraordinary team.  At Life Choice Hospice we make a strong effort to empower our team members to perform excellence in care through realistic ratios that enable you to spend quality time with patients.

This position will serve our patients in the region surrounding Plymouth Meeting, PA.

We are currently seeking a Hospice Marketing Account Executive (HMAE) to coordinate the marketing efforts and activities within their designated territory.  The HMAE provides for the development of contractual relationships, manages and nurtures ongoing relationships with external partners in care, and seeks to build partnerships with other care providers that allow the organization to grow and expand services to patients in diverse settings.

Position Responsibilities

The HMAE has the following responsibilities:

  • For the overall growth in the HMAE’s territory.
  • For identifying and securing new business to meet the company’s census goals.
  • To have a fully understand of the contract process
  • Oversees the marketing efforts within their assigned territory
  • Actively participates in Marketing Department meetings to ensure collaboration within the Marketing team.
  • Establishes and directs the marketing plan within their territory.  Communicates that plan to their supervisor.
  •  Determines the primary decision makers in their territory and works to establish a relationship and build the brand of LCH as the preferred provider and achieving goals.
  • Documents efforts and contacts according to the method established within the department.

  5. Actively participates in Marketing Department meetings to ensure collaboration within the Marketing team.
  6. Establishes and directs the marketing plan within their territory.  Communicates that plan to their supervisor.
  7. Determines the primary decision makers in their territory and works to establish a relationship and build the brand of LCH as the preferred provider and achieving goals.
  8. Documents efforts and contacts according to the method established within the department.
  9. Communicates issues to LCH clinicians that are relevant in the performance of clinical care or of relevance to the relationship with the facility staff.
  10. Acts as a liaison between LCH and  facility staff, facility administration, other external publics, or physicians.
  11. Maintains supply of collateral  materials needed for effective distribution.

Requirements
Experience
  • Minimum of two years experience as a HMAE 
  • Territory management in any type of sales environment.
Education

Must have min. of  Associates Degree.

Special Qualifications
  • Must be highly motivated, energetic and work interdependently.  
  • Requires basic time management and organizational skills, strong interpersonal relationship skills, and must be supportive of organizational philosophy/policies.
  • Possesses a valid and current driver’s license and automobile insurance.
  • Must have reliable transportation

Director of Business Development

Responsibilities
  • Oversee the strategy and design of marketing initiatives
  • Develop initiatives to enhance the Life Choice brand in the marketplace
  • Develop and lead a growing team of Account Executives
  • Create and perform educational presentations to external publics on the quality and benefits of the Life Choice program
  • Identify potential business opportunities in new markets
  • Develop new business contracts while improving production with existing business partners
  • Develop metrics, processes and analytics to create a data and customer research driven decision-making culture   
  • Identify, evaluate, and develop new business opportunities to further Life Choice Hospice’s mission and capabilities
  • Support management team in crafting long-term strategies for Life Choice Hospice’s key growth areas
Requirements
  • 5+ years of sales and marketing in the healthcare industry is a must
  • Experience specifically with hospice is strongly preferred
  • Experience with  long-term care is preferred
  • Superior interpersonal, relationship building, presentation and negotiation skills
  • Ability to travel throughout the territory 50% or more of the time
  • Reliable, insured transportation and a valid driver's license
