Social Services

At Life Choice Hospice, we provide a full spectrum of social and counseling services to our patients and their families. When dealing with a life-limiting illness, patients and families are faced with numerous challenges and are forced to make difficult decisions. Our team of qualified social workers offer a range of practical and emotional support services that will help guide them through this journey.

Our social workers can supply the information and resources needed to make informed decisions, partner with the Spiritual Care team to provide bereavement services or simply act as an advocate for the needs and rights of the patient and their family. The social worker assesses each patient and family and then meets with the entire care team to formulate an individualized care plan designed to meet their individual goals.

Some of the services that our social workers are responsible for include:

  • Attending to the practical and counseling needs of both the patient and the family based upon the initial and ongoing assessments
  • Serving as a liaison with community agencies (Department of Human Services, Department of Aging, Public Aid Office)
  • Assisting the family in finding services to address financial needs and legal matters (Power of Attorney, Advance Directives)
  • Providing counseling related to patient and family communication
  • Assessing patient and family anxiety, depression and caregiver stress
  • Providing anticipatory grief and bereavement support
