General Guidelines
The specific eligibility criteria are provided by the Centers for Medicare and Medicaid Services (CMS).
Hospice appropriate patients may exhibit some or all of the following:
- Over decline in condition
- Recurrent infections such as pneumonia, sepsis or upper urinary tract
- Progressive weight loss
- Increasing hospitalizations
- Progressive decline in mental abilities or functional status
- Multiple co-morbidities
Who would benefit from Life Choice Hospice services?
- Patients who are no longer receiving curative therapy
- Patients with a life limiting expectancy
