At Life Choice, our philosophy challenges us to go beyond the standards…to provide specialized programs tailored to the individual needs of our patients and their loved ones. From special care programs for diagnosis, to cultural programs, we strive to serve the special needs of every patient.
Recently, we added two new specialized programs to our services: Connections Pre-Hospice Care Program and Tapestries, a specialized Alzheimer’s Hospice Program.
Connections by Life Choice provides supportive services to individuals and families who are facing a serious or chronic condition causing a limited life expectancy, but who are not yet physically or psychologically ready for hospice. We understand that this is a difficult time and know that challenges will arise. There are endless questions that need to be answered and countless medical options to consider.
