Connections by Life Choice is a new program designed to provide supportive services to individuals and families who are facing a serious or chronic condition causing a limited life expectancy, but who are not yet physically or psychologically ready for hospice. We understand that this is a difficult time and know that challenges will arise. There are endless questions that need to be answered and countless medical options to consider.
Our Connections offers compassion and support to any individual or loved one facing a chronic or life-limiting illness. This program is appropriate during and after the completion of medical treatments that are focused on cure. Connections is designed to ease your concerns by answering your questions, allowing you to make more informed decisions about treatments, care and the next steps in your journey. It is a program offered at no charge.

Life Choice Hospice (PA and NJ) provides notice to its public that when an individual has any concerns about patient care and safety in the organization, that the organization has not been addressed, he or she are encouraged to contact the organization’s management.
