Connections by Life Choice is a new program designed to provide supportive services to individuals and families who are facing a serious or chronic condition causing a limited life expectancy, but who are not yet physically or psychologically ready for hospice. We understand that this is a difficult time and know that challenges will arise. There are endless questions that need to be answered and countless medical options to consider.

Our Connections offers compassion and support to any individual or loved one facing a chronic or life-limiting illness. This program is appropriate during and after the completion of medical treatments that are focused on cure. Connections is designed to ease your concerns by answering your questions, allowing you to make more informed decisions about treatments, care and the next steps in your journey. It is a program offered at no charge.

Our services may include:

• In-person evaluation by a member of our care team to assess your needs
• Support from Life Choice team members over the telephone
• Answers to your questions regarding care and medical options
• Help in understanding diagnoses and medical concerns
• Assistance in helping you to make informed decisions about care and medical choices
• Help communicating concerns or changes in condition with your doctor
• Assistance with referrals to appropriate community resources
